This is probably an incomplete answer since I was never involved in the distribution side, but in the old days, the major labels ate the cost of defects and returns and probably pushed back against the pressing plants if there were defective press...

Norman- have you heard the old Horzu Die Beatles -2? It is a hoot, very spare, hard panned left and right, no additional effects. I don't have original Parlophones to compare, but I thought the mono box on vinyl was good.
