Tube Integrated in Home Theater?
Don't turn tubes on/off 10 times a day. The heating up and cooling off will shorten the tube life more than just leaving them on. If you know that you will be listening again in the next 4 hours or so, just leave them on. Good ventillation will help. 
Being given 2 custom systems but what to ask for?
Wow! What city are you in?Budget a professional installer into the list of gear. If you are a "newbie" this is just as important as picking the right gear.Without seeing the rooms that these will be in, it is tough to recommend the right stuff. So... 
Considering referb on Harman Kardon A230 tube amp
cleaning it up and getting it working properly is worth it. Spending more than $150 on it probably isn't worth it. A fisher 101, scott 299 or similar will probably beat it. 
Should I add a separate amp?
I think you'd have to spend a bundle to notice a difference. You don't need more power but who can argue with a Lexicon or Sunfire vs the built in Denon. However, you'd be looking at 7 more interconnects also!

Output tubes should last 1000-2000 hours as long as you don't turn them on/off too often.Gain/driver tubes last 5000-10,000 hours. You do the math with your listening habits.

First, figure out one at a time. I'm totally for 2 independent systems.You need to look at the room where they will be used, how often they will be used and how simple they are to use. Obviously, you need to keep them simple or you won't ever turn... 
cheap tube integrated for 4 ohm speakers
A hybrid (jolida) may be what you need. 4ohms is fine with most tube amps, its the 89db driven with authority that's not so good. You really should look at 100wpc or greater amps. 
Help with Pre-Amps
Some pre-amps have great phono stages built in (AR-6 and more). However, most pre's that I've played with have second rate phono stages.I'd say keep your phono stage. Try different pre-amps. I'd go with one with fewer "gadgets" will probably sound... 
tube amp wattage questions/psb golds
20 Watts isn't enough for Golds. They sound great and are moderately efficient but really like more power than 20 wpc. Those big woofers get hungry. If a big tube amp isn't in your future, consider a Class A or Class A/B solid state amp. You will ...

TLP great pre-amp with built in tuner. This is the same quality as the $3000 pre/pro/tuners without the surround junk. 
CD players with volume control?
Not all volume controls are created equal...beware of many units with awful volumes that either go up in large steps or ones that are simply too hard to adjust in fine amounts. I tried this and ended up with a creek obh-12 to attenuate the volume.

Bi-amping
Markphd is so right. Sell the Carver and put that money into upgrading the AR. That amp running well is better than mixing in the mediocre sound of the carver. 
phono cartridge questions
Great budget cart but you should try a better one on one of your tables to see what you are missing. You won't have to tweak the phono pre's if there isn't any roll-off. Try even an Ortofon or Goldring in the $300 range and you will see for yourself. 
Suggestions for good Preamp/tuner
The B&K sounds great but had a really quirky remote. Get an Anthem TLP.
