ONT connection issue

I have an ONT  OptiXstar K662c which was installed in my house. It worked well a few years. But recently, the speed of connection became slow as I connect it with the wrong cable interface. I forget what is the version I need to change my account information to reset it.  The problem is that I forget the code. How do I find the code?
