There are good free apps for this on Itunes. That's how I picked up my own hearing loss.

From my experience I would say that these apps and websites offer a very basic snapshot of one's hearing. I don't know if I would make any medical decisions based on their outcome. You're going to be limited by the headphones or speaker on the device you're using. My work uses a product called SHOEBOX Audiometry. They use an iPad, but they also include calibrated headphones.
